Independent Facilitation

Independent Facilitation

You have the right to “independent facilitation” of the Person-Centered Planning (PCP) process. This means that you may request that someone other than CMH staff conduct your planning meetings. You have the right to choose from available independent facilitators. Independent facilitator is neutral and also has no financial interest in the outcome of the supports and services outlined in the PCP.

Why should you use an Independent Facilitator?

An Independent Facilitator:

  • identifies a person’s strengths and needs
  • makes sure the individual is heard and understood
  • keeps the meeting on track
  • locates available community resources and services
  • uses tools to uncover a person’s plans
  • records and reports meeting discussions
