About This Service

Assertive Community Treatment (ACT) is an evidenced-based model with a therapeutic set of intensive clinical, medical, and psychosocial services provided by a mobile multi-disciplinary team. Services are individually tailored and include case management, psychiatric services, counseling/psychotherapy, housing support, substance use disorder treatment, and employment and rehabilitative services. Services are provided in an individual’s home or within the community and include availability of multiple daily contacts with 24/7 crisis availability. 

ACT is provided to individuals with a primary diagnosis of serious mental illness (SMI) and who, without ACT, would require more restrictive services and/or settings. Services are targeted to individuals demonstrating acute or severe psychiatric symptoms seriously impairing the individual’s ability to function independently, and whose symptoms impede the return of normal functioning as a result of the SMI diagnosis. Qualifying SMI diagnoses may include Schizophrenia, Bipolar Disorder, and Major Depressive Disorder with Psychosis.
