Overview

As a Supports Coordinator Assistant you will perform the traditional case management functions of assessment, planning, advocacy, monitoring, and linking & coordination of services while collaborating with other staff in assessing the needs of the individuals we serve and developing a person centered and family focused treatment plan.

This will include supporting the dreams, goals, and desires of the individuals we serve to optimize their independence, promote their recovery, and assist in the development and maintenance of natural supports. Your community connections and desire to help others will serve you well in this role.

Hours: Full-Time (preferred) hours are generally Monday-Friday 8am-5pm. Part-time hours are a minimum of 24 hours per week during the Monday-Friday 8am-5pm timeframe.

Must have: High School Diploma (advanced degree preferred), and experience with behavioral health.

Salary: $16.71-$23.56/hr

We are currently looking for someone to work with our Older Adult population (55+ yrs), our Applied Behavior Analysis (ABA) autism population (up to 18 yrs), or our Children’s Services team (up to 18 yrs).
