Overview

As our Older Adult Services (OAS) Nurse, you will assess and case manage aging individuals with mental health concerns. This will include developing person-centered plans of care that provide services and supports to our individuals, advocating for the individual, educating them on their conditions and medications, and working alongside a team of clinicians to provide a variety of person-centered services. Your experience with the geriatric population and/or mental illness will serve you well in this position.

Hours – Full-Time hours are generally Monday-Friday 8am-5pm with a rotational on-call.

Must have – Registered Nurse license in the state of Michigan and a valid driver’s license.

Salary – $25.61 - $35.37/hr

 

What is Older Adult Services?

Older Adult Services include assessment, treatment, and consultative services for individuals with serious mental illnesses, including those co-occurring with substance use disorder, who are 55 years of age or older. Service modality includes, but is not limited to, targeted case management, crisis intervention, advocacy services, supportive counseling, linking, coordinating, and monitoring as defined by the Medicaid Manual and NeMCMHA Level of Care Guidelines. Services occur in the community, the Agency, the individual’s home, foster care home, or nursing home.
