Overview

As our Older Adult Services Case Manager you will assess and manage individuals over the age of 55 receiving mental health services. You will work to develop person-centered plans of care which provide them with services and supports, advocate for them regarding their conditions and medications, and work alongside a team of clinicians to provide person-centered services. Your experience with the older adult population will serve you well in this role.

Hours - Full-Time hours are generally Monday-Friday 8am-5pm with a rotational on-call.

Must have – Bachelor’s degree in Social Work or be a Registered Nurse, and have licensure according to state guidelines. A valid driver’s license is also required.

Salary – Bachelor’s: $20.78 - $28.64 | Registered Nurse: $25.61 - $35.37
